     Passport size photo of minor and guardian
     Photocopy of ID/Passport of guardian
     Photocopy of Birth Certificate/Birth Notification/
    Baptism Card
  3. CORPORATE MEMBERSHIP ACCOUNT
     Passport size photo of signatories
     Photocopy of ID/Passport of all signatories
     List of all group members
     Group By-laws/Constitution
     Copy of renewed registration certificate
     Minutes of the last meeting
